I represent I am the Participant or the parent or legal guardian of the Participant(s) named above, or I have obtained permission from the parent/legal guardian of the Participant(s) to execute this agreement on their behalf, and that all information stated above and herein is true. I am aware that dance is a physical activity and that injury or accident may occur, and Participant(s) (and I) assume all risk related to or arising out of participating in Stepz activities and represent that Participant(s) are in good health and suffer from no physical impairment and carry appropriate medical insurance. I agree to inform Stepz and its instructors of any physical limitations that may prevent full participation. I understand that Stepz has not and will not render any medical services including any medical diagnosis. I agree that the Participant(s) named above and I shall comply with all stated terms, rules, and verbal instructions as conditions for participation or presence in any Stepz activities. I am aware that there are inherent risks associated with participation in Stepz programs, and I, on behalf of myself and the Participant(s) named above, knowingly and freely assume all such risk, both known and unknown, including those that may arise out of the negligence or other misconduct of other participants or Stepz; and, I, for myself and the Participant(s) named above, and our respective heirs, assigns, administrators, personal representatives, and next of kin, hereby release and hold harmless, Stepz, its affiliates, managers, officers, directors, shareholders, agents, instructors, employees, other participants, and related agencies (collectively, the “Released Parties”) from and against any and all claims, injuries, liabilities or damages arising out of or related to our presence and/or participation in any and all Stepz programs, activities, or facilities.I agree that neither I nor any Participant(s) will sue any Released Parties for any claim, demand, cause of any action of any kind whatsoever for, or on account of any death, personal injuries or any other damages sustained. I understand that Participants may decline to participate in any Stepz activity and I understand that Stepz instructor(s) have varying levels of expertise and have not been certified as "experts" (or anything else) in any professional manner. I also promise not to use the choreography or similar steps to teach a dance without the written permission of Stepz’ CEO, Deepika Jain. I also give permission for Stepz to take photos or other recordings of me and any other Participant(s) to use for purposes of promoting Stepz. I understand that STEPZ may charge me for the entire amount of the class if I reserve a spot in the class and then do not participate. I also understand that refund of all or part of my class fee for any reason may be granted or denied by STEPZ at its sole discretion. A minimum number of registrations are needed to run each class and if the class is not filled, STEPZ may cancel the class with short notice and provide the student with an alternate class / alternate session. If any part of the above is determined by any court of competent jurisdiction to be invalid, unlawful, void, voidable or unenforceable at all, the remainder of the foregoing shall continue to be in effect, valid and enforceable to the fullest extent permitted by law.

I represent I have read the foregoing in its entirety and fully understand all terms, and I voluntarily agree to all terms and conditions stated above.
